It cost us $53.25 and some 4 weeks of effort to get a copy. We've reproduced it here so that anyone needing it can download it free of charge. The words are identical. The formatting is close but not exact. We used a Macintosh Computer and the Pages word processing program along with an OCR program to scan and reformat it into it's current PDF format.
